Records Request Policy
How to request a copy of your records and how OuiHealth processes those requests.

1. Who May Request
Clients, parents or guardians of minors, and authorized legal representatives may request copies of records pertaining to the client. Caregivers and staff may request copies of their own personnel records.
2. How to Request
Submit a written request to ouihealthmanagement@gmail.com or by mail to [Mailing Address — Albany, GA]. Requests should identify the requestor, the subject of the records, the records sought, the preferred format, and the delivery method.
3. Verification
We verify identity and authority before releasing records. Additional documentation may be required, including government-issued ID and proof of legal authority.
4. Response Time
We respond to records requests within the timeframes required by HIPAA and Georgia law, generally not later than thirty (30) days, with one permitted extension where applicable.
5. Fees
We may charge a reasonable, cost-based fee for copying, mailing, or preparing summaries, consistent with applicable law.
6. Denials
Where access is limited or denied under applicable law, we will provide written notice of the basis and any appeal rights.
