The rights and responsibilities of clients receiving services from OuiHealth.

1. Client Rights
- To be treated with dignity, respect, and consideration for personal values and beliefs.
- To receive care free from discrimination, abuse, neglect, exploitation, and harassment.
- To privacy and confidentiality of personal and health information.
- To be informed about services, care plans, expected outcomes, and possible risks.
- To participate in decisions about your care and to refuse services to the extent permitted by law.
- To receive a written Service Agreement and a copy of these policies.
- To voice complaints and recommend changes without retaliation.
- To receive care delivered by qualified, screened, and trained caregivers.
- To timely notice of changes affecting your care, including caregiver substitutions where feasible.
- To request access to and amendment of your records as provided by law.
2. Client Responsibilities
- To provide accurate information about your health, condition, contacts, and circumstances.
- To treat caregivers and staff with respect and to provide a safe environment.
- To follow agreed-upon care plans and to communicate changes affecting care promptly.
- To meet financial obligations under the Service Agreement and Payment Terms.
- To respect the privacy and confidentiality of OuiHealth information and systems.
