HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Obligations

OuiHealth LLC is required by law to (a) maintain the privacy of your protected health information ("PHI"), (b) provide you with this Notice of our legal duties and privacy practices with respect to PHI, (c) notify you following a breach of unsecured PHI, and (d) abide by the terms of the Notice currently in effect.

2. Uses and Disclosures for Treatment, Payment, and Operations

• Treatment: coordinating and managing your care with internal and external providers.
• Payment: billing, claims, eligibility, and collection activities.
• Healthcare Operations: quality improvement, audits, training, licensing, accreditation, and business management.

3. Other Permitted or Required Uses and Disclosures

• When required by law, public health activities, abuse or neglect reporting, health oversight, judicial or administrative proceedings, law enforcement, coroners, organ donation, research with safeguards, and to avert serious threats to health or safety.
• To business associates under written agreements that require them to safeguard PHI.
• To family members, friends, or authorized representatives involved in your care, where permitted.

4. Uses and Disclosures Requiring Your Written Authorization

Other uses and disclosures, including most uses or disclosures of psychotherapy notes, marketing communications, and the sale of PHI, require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

5. Your Rights

• Right to request restrictions on uses or disclosures.
• Right to request confidential communications by alternative means or locations.
• Right to inspect and copy your PHI (subject to limited exceptions).
• Right to request an amendment of your PHI.
• Right to receive an accounting of certain disclosures.
• Right to receive a paper copy of this Notice on request.
• Right to be notified of a breach of your unsecured PHI.

6. Complaints

You may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint. Submit complaints to OuiHealth Privacy Officer, ouihealthmanagement@gmail.com, (229) 234-2649.

7. Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you. The current Notice will be posted on our website and available upon request.
