Complaint Submission and Resolution Notice
How to raise concerns and how OuiHealth investigates and resolves them.

1. Raising a Concern
You may raise concerns or file a complaint at any time without retaliation. Contact us by phone at (229) 234-2649, by email at ouihealthmanagement@gmail.com, or by mail to [Mailing Address — Albany, GA].
2. Acknowledgement
We acknowledge complaints promptly, generally within three (3) business days, and assign a designated point of contact.
3. Investigation
We investigate complaints fairly and confidentially, gathering information from involved parties and reviewing relevant records.
4. Resolution
We aim to resolve complaints as promptly as the circumstances allow. You will receive a written summary of findings and any action taken, where appropriate.
5. External Complaints
You may also file a complaint with the Georgia Department of Community Health, Healthcare Facility Regulation Division, and with the U.S. Department of Health and Human Services for HIPAA matters. We will provide contact information on request.
6. Non-Retaliation
OuiHealth prohibits retaliation against any person for raising a concern or filing a complaint in good faith.
